For the chiropractic paperwork, you may fill out the form on your computer or tablet and the information will automatically upload to our computer system and be ready for your appointment. If you would prefer you can print a copy of the paperwork, complete it and bring it to your appointment with you. You can also complete the forms at our office on paper or an ipad.
For massage therapy, you will need to print the paperwork, complete it and bring it with you to your appointment or complete the forms in office.
We acknowledge that the forms may seem lengthy, but they will assist us in understanding your entire health history and important concerns.

- How do I book an appointment?
In order to schedule an appointment for chiropractic care, massage or bodywork please call 734-677-1900.
- What are the fees for your services?
All appointments are payable by cash, check or credit card.
Fees will vary according to time spent with physician, complexity of your problems and type of treatments rendered.
Bodywork / Massage Therapy
While gratuities are a standard practice at a salon, spa or clinic where therapists do not set their own rates, gratuities are not necessary for our services at CCBT. We set our rates appropriately, and are honored to be a part of your health team.
- Is CCBT a participating provider with my health insurance?
Insurance policies vary widely in what is and is not a “covered” benefit. Chiropractic coverage is provided under many insurance policies including BCBS, Medicare, Aetna, MESSA and many others. We do bill all insurance companies for chiropractic care, as a courtesy to our patients. We are participating providers with Blue Cross/Blue Shield Traditional and PPO plans. Our Financial Coordinator, Kendra, verifies each chiropractic new patient’s insurance information and will explain your particular coverage.
INSURANCE COVERAGE FOR MASSAGE THERAPY
We only do billing of massage therapy services for patients who are currently under the care of our staff chiropractors or who are referred through the Veterans Administration system. Clients with prescriptions from other practitioners are welcome to receive care, but will be provided receipts and must do their own billing.
- Is there insurance coverage for Veterans at your office?
Yes. With a prescription from your VA doctor and authorization from the VA, veterans are eligible to receive chiropractic treatment. For more information, call our office 734-677-1900 or your local VA.
- What is your Cancellation Policy at CCBT?
There is a 24-hour cancellation policy. If your appointment is missed you will be charged the full amount of that service. However, if you miss an appointment due to illness or an emergency there will be no charge.
- What kind of attire is appropriate for my appointment?
If seeing a chiropractor, we request you wear or bring loose fitting clothing such as warm up pants, shorts, T-shirts, etc., as your visit will involve structural examination and treatment.
If you are seeing a massage therapist, the best way to receive a massage or other bodywork is to be unclothed. Our practitioners suggest you undress to the level of your comfort and you will be appropriately draped at all times.
- May I purchase a gift certificate for services provided at your clinic?
Gift certificates are available for all massage and chiropractic services at CCBT. Contact our office at (734) 677-1900 and we’ll be happy to make arrangements for purchase.
